Pulmonary Fibrosis Learning and Self-Management Program (FiLiP)

FiLiP aims to develop and evaluate a blended education and self-management program for patients with fibrotic interstitial lung disease (FILD). Self-management programs support people with chronic illnesses in managing their condition more effectively and thereby improving quality of life. The goal of FiLiP project is to create a modern, digital training and self-management program for individuals with FILD that can be used in both inpatient and outpatient rehabilitation or care settings. The project applies co-creation and human-centered design approaches to achieve this goal.

The first phase involved patient and expert interviews, as well as a Delphi study, to understand patients’ lived experiences and define the educational and self-management content, using a patient-centered co-development approach.

In the second phase, self-management modules were developed using the IDEAS and ADDIE frameworks. These modules were refined through patient-centered testing, including cognitive debriefings and usability testing, to ensure they meet patients’ needs.

The current third phase focuses on evaluating the complete program, which follows a blended learning approach. The evaluation examines feasibility in terms of treatment-related outcomes (satisfaction, applicability, acceptance, adherence) and study-related outcomes (recruitment success, dropout rates). Testing takes place in both an inpatient rehabilitation clinic (Berner Reha Zentrum) and an outpatient medical consultation setting (University Hospital of Bern). In addition, an embedded qualitative substudy explores barriers and facilitators for national implementation and long-term sustainment of the program.

The project is a collaboration between the Epidemiology, Biostatistics and Prevention Institute (University of Zurich), Berner Reha Zentrum, University Hospital of Bern, and Zurich University of Applied Sciences (ZHAW).
